Appointment Booking and Cancellation Policy

Purpose

Mansfield Cosmetic Surgery Center is committed to providing exceptional care and personalized attention to every patient. Because appointments with Dr. Michael L. Thornton and our licensed medical aesthetician are in high demand, we reserve dedicated appointment times for each patient. This policy helps us manage our schedule, respect the time of our patients and providers, and make appointments available to others who are waiting to be seen.

Consultation and Appointment Booking Deposits

1) Cosmetic Surgery Consultations: $125 Booking Deposit

A $125 booking deposit is required to schedule an initial face, breast, or body cosmetic surgery consultation with Dr. Michael L. Thornton.

During your consultation, you will meet personally with Dr. Michael L. Thornton, a double board-certified cosmetic surgeon and Diplomate of the American Board of Cosmetic Surgery. Your appointment includes a comprehensive evaluation, a discussion of appropriate surgical options, and an individualized treatment plan based on your aesthetic goals. Following your consultation, our office will provide a personalized surgical quote outlining the proposed procedure and associated costs.

The $125 booking deposit is nonrefundable and is collected at the time of scheduling the consultation appointment. If you proceed with a cosmetic surgical procedure that is recommended by Dr. Thornton, we will apply the full $125 booking deposit as a credit toward the surgeon’s fee listed on your surgical quote.

The booking deposit does not obligate you to undergo surgery, guarantee surgical candidacy, or guarantee a particular surgical outcome. The deposit is not redeemable for cash and cannot be transferred to another patient.

2) Injectable Consultations and Treatments: $50 Booking Deposit

A $50 booking deposit is required for first time patients scheduling an appointment with Dr. Thornton for evaluation and consultation regarding Botox, Xeomin, or dermal filler treatments.

During your visit, Dr. Thornton will assess your candidacy, discuss appropriate treatment options, and develop a personalized treatment plan. Same day treatment is not guaranteed and may require a separate appointment.

3) Medical Spa Consultations and Treatments: $50 Booking Deposit

A $50 booking deposit is required for first time patients scheduling a medical spa consultation or initial treatment with our licensed medical aesthetician, Chelsea Jarbo-Adams.

These booking deposits are nonrefundable and are collected when the appointment is scheduled. For an initial treatment appointment, the $50 deposit will be applied toward the cost of the treatment performed at that appointment. For consultation appointments, the deposit will be applied toward an eligible treatment subsequently scheduled with our practice.

Booking Deposit Agreement

By scheduling an appointment and submitting the required booking deposit, you acknowledge and agree to the terms of this policy.

Your payment reserves a designated appointment time with our providers. All booking deposits are nonrefundable from the time of payment, including when you elect not to proceed with the consultation or treatment.

Booking deposits may be transferred to a rescheduled appointment when you provide the required advance notice, as described below.

Appointment Reminders

As a courtesy, we will attempt to remind you several business days before your appointment by telephone, text message, or email. Appointment reminders are not guaranteed.

You remain responsible for attending your scheduled appointment or notifying our office of any cancellation or scheduling changes within the required time.

Cancellation and Rescheduling Notice

We require at least 24 hours advance notice to cancel or reschedule an appointment.

The 24 hour notice period refers to the same clock time on the preceding business day. For Monday appointments, notice must be received by the same clock time on Friday. Federal holidays and days when our office is closed do not count as business days.

If you provide the required notice and wish to reschedule, your existing booking deposit will be transferred to your new appointment without an additional booking deposit.

If you elect to cancel your appointment entirely, your original booking deposit remains nonrefundable.

If you cancel with less than the required notice or fail to attend your appointment, you will automatically forfeit the original booking deposit and will be required to pay an additional $50 booking deposit before scheduling another appointment.

Any additional booking deposit collected for rescheduling is also nonrefundable and will be applied toward the applicable consultation or treatment under this policy.

How to Cancel or Reschedule

Please contact our office within the required notice period so we can offer your appointment time to another patient.

Your cancellation or rescheduling request is recorded when our office receives your message through one of these approved communication channels.

Late Arrivals

If you arrive 10 minutes or more after your scheduled appointment time, we may shorten your scheduled visit or require you to reschedule, depending on the provider availability and the needs of other scheduled patients.

If we cannot accommodate your appointment because of a late arrival, we may treat the appointment as a same day cancellation.

Your original booking deposit will remain nonrefundable, and an additional $50 booking deposit may be required to reschedule.

Fees for Missed or Late Cancelled Appointments

Patients who miss an appointment or cancel without the required advance notice may incur the following charges:

Established Patients: A $25 rescheduling fee may be assessed at the provider’s discretion.

New Patients: A $50 rescheduling fee may be assessed before another appointment is scheduled.

For appointments requiring an advance booking deposit, the original deposit is forfeited following a missed appointment or late cancellation. An additional $50 booking deposit may be required to schedule a replacement appointment.

Our office will explain any applicable fees before confirming a new appointment.

Exceptions and Special Circumstances

We understand that unexpected circumstances can occasionally prevent patients from attending scheduled appointments.

Please notify our office as soon as possible if you experience sudden illness, injury, hospitalization involving yourself or a dependent, contagious illness, unsafe weather, widespread utility outages, bereavement, or another serious unforeseen emergency.

Our office may waive rescheduling charges or permit the transfer of an existing booking deposit when circumstances warrant an exception. We may request reasonable documentation. All exceptions remain at the discretion of Mansfield Cosmetic Surgery Center.

Procedures and Surgery

In office procedures and outpatient surgical bookings may require additional deposits, advance notice, or separate cancellation agreements.

These appointments are governed by the specific financial and cancellation policies provided when the procedure or surgery is scheduled.

Policy Acknowledgment

By scheduling an appointment and paying a required booking deposit, you acknowledge that you have reviewed and accepted the terms of this Appointment Cancellation and Booking Deposit Policy, including the nonrefundable nature of the deposit, applicable rescheduling requirements, and potential additional charges.

Policy Updates

Mansfield Cosmetic Surgery Center reserves the right to update this policy periodically. The current version will be available on our website and at our front desk.
